Lisa O’ Hara, Nurse Consultant Palliative and End of Life Care, Sussex Community NHS Foundation Trust

Lisa O’Hara leads a team at Sussex Community NHS Foundation Trust to provide vital, compassionate end-of-life care and symptom management for patients in their own homes, care homes, or community beds.

The community nursing service is focussed on symptom management, holistic support, and care coordination between hospital, hospice, and home. Lisa actively champions dignity in dying and works tirelessly to unify end-of-life education across Sussex.

Working in a small team with colleagues from Sussex Community and University Hospitals Sussex, Lisa has helped develop the Comfort Tracker – a remote monitoring app that enables proactive, integrated care for patients who want to live comfortably, and die comfortably, at home.

“The Comfort Tracker app helps to reduce unnecessary admissions, empowers patients, and helps us deliver the best patient care at home. In addition, it improves carer understanding of what to expect when someone dies. All this ties into the NHS 10 Year Health Plan for England: using digital technology to improve patient care and moving care out of the acute and into the community setting where appropriate.”
