Supported self-management

Supported self-management (SSM) is part of the NHS Long Term Long-term Plan’s commitment to make personalised care business as usual across the health and care system. It proactively identifies the knowledge, skills and confidence (‘activation’) people have to manage their own health and care, using tools such as the Patient Activation Measure® (PAM®).

Health and care professionals tailor their approaches to working with people, based on the person’s individual assets, needs and preferences, as well as taking account of any inequalities and accessibility barriers, and so working in a personalised way based on ‘what matters’ to the person. It also means ensuring approaches such as health coaching, peer support and self-management education are systematically put in place to help build knowledge, skills and confidence.

Patient activation

Patient activation describes the knowledge, skills and confidence a person has in managing their own health and care.

Patient Activation Measure® (PAM®)

Patient Activation Measure licences are available from NHS England as part of the Supported Self-management component of the Personalised Care programme.

Health coaching

Health coaching helps people gain and use the knowledge, skills, and confidence to become active participants in their care in order to reach their health and wellbeing goals.

Peer support

Peer support is a range of approaches through which people with similar long-term conditions support each other to better understand the conditions and aid self-management.

Self-management education

Self-management education is any form of formal education for people with long-term conditions focused on helping them to manage their own health care effectively.
