Introduction to Cardiovascular Disease (CVD) Prevention

CVD continues to be one of the main causes of morbidity and mortality and contributes towards health inequalities. The NHS Long Term Plan has identified CVD as one of its priority areas for action over the next ten years. Prevention of cardiovascular disease is a key priority in London. However, many adults living with Atrial Fibrillation (AF), hypertension (HTN) and Familial Hypercholesterolaemia (FH) remain undiagnosed. Without detection and medicine management, these conditions can increase the risk of having a stroke or heart attack, leading to higher mortality rates or greater disability. Ambitions for improving the detection and treatment of the 3 high risk conditions have been set nationally.

The CVD vision for London has been drafted by local system partners. Our regional data has been used to make sure the CVD ambition for London is aspirational but also achievable and clinically credible.

The proposed CVD vision and ambition for London state that by 2023 we will:

  • Save an additional 400 lives each year by reducing the number of heart attacks and strokes
  • Empower Londoners to take control of their circulatory health
  • Work together with all our system partners to find and optimally treat Londoners with any of the three high risk conditions (high blood pressure, high cholesterol and atrial fibrillation).

Pan London ambitions:

  • Detect 85% of expected population with AF without exceptions
  • Protect 87% of all people with AF and a CHA2DS2Vasc score of 2 or more with anticoagulant therapy
  • Perfect anticoagulant pathways to ensure 90% of people with AF are considered for anticoagulant therapy within one week of referral
  • Detect 80% of expected population with hypertension
  • Protect 80 % of all people with known hypertension by controlling BP to 140/90mmHg
  • Find 25% of the expected population with FH.

The London Cardiovascular Disease Prevention Partnership (CVDPP) wants to help systems meet the regional requirement to produce a plan showing how, by the end of 2023, they will deliver on the vision and ambitions for CVD Prevention in London through the provision of this resource.

CVD Prevention Partnership Resource Pack

Regional level coordination for CVD prevention will promote clinical engagement and consensus amongst providers, commissioners and service users for systematic adoption of evidence-based interventions at pace and scale. It is to this end that the CVDPP has constructed a resource pack providing a multitude of diverse evidenced-based interventions, pilots and studies from across the country that have a promising or proven impact on CVD prevalence and prevention. This will provide NHS planning and commissioning bodies with the necessary tools to select targeted, evidence-based interventions to tackle cardiovascular disease across London.
